We currently serve 74 low income elders yearly. Elders are referred by other social service agencies, HUD building managers and neighbors. Elders are assessed using a multi-functional assessment to determine their unmet needs. We provide Case Management for three months and follow up services for three additional months, however many of our participants require more than 6 months of case management in order to obtain appropriate levels of assistance. Case management services may include assisting the participant with applications for subsidized housing, special transportation, applying for additional services, home delivered meals, medical insurance, electric and telephone subsidies and advocacy. Our goal is to obtain the necessary services for the elder that will allow him/her to continue living independently in their homes.
